Qualifications

• BSN is required by the job posting.
• Master’s degree in nursing, education, public health, or related field preferred.
• Experience with diabetes education and chronic disease management is strongly preferred.

Responsibilities

• Perform nursing and educational assessments for patients with diabetes and related conditions.
• Develop and update individualized diabetes education plans based on provider directions.
• Educate patients on medications, glucose monitoring, and diabetes technologies.
• Collaborate with a multidisciplinary team to coordinate education and follow-up care.
• Contribute to quality improvement and population health initiatives within the Diabetes Clinic.
