Job summary

Tuba City Regional Health Care Corporation is seeking a qualified Registered Nurse to provide diabetes self-management education to patients, coordinating group education classes and case managing diabetes care for both adults and children.

Requirements include a Bachelor’s degree in Nursing, an active RN license, and BLS. Two years of RN experience, preferably with type 2 diabetes, is expected. Proficient in MS Office and healthcare documentation is needed.

Qualifications

• Bachelor's degree in Nursing required.
• Must have current RN license to practice in US territory.
• Must have Basic Life Support (BLS) certification.

Responsibilities

• Provides inpatient and outpatient diabetes self-management education.
• Performs patient needs assessments and tailors education accordingly.
• Delivers education in individual and group settings using standard curricula.
• Documents services to ensure third-party reimbursement.
• Applies ADA and IHS Diabetes Standards of Care.
• Uses motivational interviewing and behavior change techniques.
• Follows up with referred patients through case management.
