Job summary

A regional health services provider in Wilmington, NC seeks a DHH Regional Care Coordinator to provide case management services to Deaf and Hard of Hearing individuals. The ideal candidate will possess a Master’s degree in a related field and experience with mental health challenges. Responsibilities include planning consumer transitions and maintaining regular contact with clients and treatment teams. A mastery of sign language is essential. Qualifications

• Master’s Degree in a related field and one year of experience serving persons with mental health or substance abuse.
• Bachelor’s Degree in Human Services with two years of experience serving the same population.
• Fluency in sign language, as per SLPI score of Advanced Plus or better.

Responsibilities

• Provides on-call/first responder services as scheduled.
• Plans and facilitates consumer transitions to other services.
• Maintains regular contact with persons served and their treatment teams.
• Assures consistent provision of quality services.
• Assess needs and provide linkages/referrals to resources to achieve personal goals.
• Represents RHA at local management entity and other meetings.
