Job summary

The Center for Human Development, Inc. in La Grande, Oregon seeks a qualified professional to identify access, coordinate and assure delivery of supports and services to individuals with intellectual/developmental disabilities.

The role includes determining eligibility, assisting families, and helping clients obtain services and benefits, with emphasis on teamwork and reliable attendance. Comprehensive benefits are provided.

Qualifications

  • Bachelor's degree in behavioral or social science or related field required.
  • Alternative: bachelor's in any field with 1 year human services experience.
  • Alternative: associate's degree with 2 years human services work.
  • Three years of equivalent training and work experience acceptable.
  • Knowledge of Oregon developmental disability services public service system preferred.

Responsibilities

  • Identify access, coordinate and assure delivery of supports and services to individuals with intellectual/developmental disability.
  • Determine eligibility for services per state guidelines.
  • Provide support and information to family members including consultation and education groups.
  • Assist clients in applying for and obtaining services and benefits.
  • Assist client with transportation if needed.
  • Assist client in community involvement.
  • Serve on advisory boards/committees related to disabilities.
  • Monitor appointments, progress and maintain case notes and reports in a timely manner.
  • Provide counseling to clients and families and respond to crises.
  • Monitor delivery of services to individuals with disabilities.
  • Work effectively as part of a team.
  • Maintain punctual and regular attendance.
  • Willingness to work flexible hours.

Job description

The Requirements:

Bachelor's degree in behavioral science, social science, or a closely related field; or a bachelor’s degree in any field and one year of experience in human services; or an associate’s degree in behavioral science, social science, or aclosely related field and two year’s work experience in human services; or Three years of equivalent training and work experience; and knowledge of the public service system for developmental disability services in Oregon.

Essential Functions:
  • Identify access, coordinate and assure delivery of supports and services to individuals with an intellectual/developmental disability.
  • Determine eligibility for services as an individual with an intellectual/developmental disability per state guidelines.
  • Provides support and information to family members including individual consultation, support/education groups.
  • Assists clients in applying for and obtaining services and benefits.
  • Assist client with transportation if needed.
  • Assist client in community involvement.
  • Serves on advisory boards and committees concerned with intellectual/developmental disabilities.
  • Monitors client appointments and progress; maintains accurate case notes, treatment plans and client files; completes reports as required in a timely way.
  • Provides supportive and problem-solving counseling to clients and their families; responds to crisis situations.
  • Monitor delivery of services to individuals with intellectual/developmental disabilities.
  • Have the ability to work effectively as a member of a team.
  • Must maintain punctual and regular work attendance
  • Willingness to work flexible hours.
