Milestone is seeking a dedicated habilitation aide to support individuals with developmental disabilities in Loves Park, IL. You will provide personal care services, follow individualized plans, and participate in daily activities and lesson plans.

Under supervision, you will monitor health, document progress, and collaborate with a team to ensure a safe, respectful environment. High school diploma or GED is required; TABE test may be required.

Qualificações

• High school diploma or GED required.
• Must pass the TABE test.
• Habilitation aide certification completed.

Responsabilidades

• Provide personal care and treatment for individuals with developmental disabilities as directed.
• Monitor health needs, document changes, and report emergencies to supervisors.
• Maintain up-to-date individual records and participate in treatment and activity plans.
• Assist with daily living skills, recreation, and community outings per plans.
• Ensure safety, confidentiality and cooperation with team members.
