Job summary

ClareMedica Health Partners, LLC is seeking an experienced Risk Adjustment Coder to review medical records, identify conditions, and code encounters in alignment with risk adjustment models.

The ideal candidate holds CPC certification (CRC preferred), with 3+ years of medical coding experience and strong knowledge of ICD-10-CM and CPT guidelines. Proficiency in EMR and MS Office is required.

Qualifications

• CPC certification required or by AAPC/AHIMA; CRC preferred.
• Minimum 3 years medical coding experience; knowledge of ICD-10-CM and CPT.
• Strong understanding of medical terminology, anatomy, and physiology.
• Accurate and detail-oriented with ability to multitask and meet deadlines.

Responsibilities

• Review medical records and coding to ensure adherence to risk adjustment models.
• Code office procedures accurately and completely.
• Ensure compliance with Federal/State regulations and coding guidelines.
• Collaborate with team to meet department objectives and production goals.
• Attend departmental meetings and participate in ongoing education.
