Job summary

AndHealth LLC in Indianapolis, IN seeks an Advanced Practice Provider with medical dermatology experience to join an onsite care team. You will diagnose, treat, and consult with patients within a collaborative, whole-person care model and coordinate with community partners.

Applicants must hold an active Indiana license as a Physician Assistant or Nurse Practitioner, with Master’s degree and certification (PA-C/NP). Experience with telehealth and EHR systems is preferred to support patient care.

Qualifications

• Master’s degree from an accredited program is required.
• Certification as PA-C or NP is required.
• Active license to practice as a Physician Assistant in Indiana (or NP) is required.

Responsibilities

• Create personalized treatment plans for dermatology patients.
• Work within a collaborative care team for patient management.
• Review and oversee care progress, coordinating with coaches and partners.
• Order and interpret labs as needed and assess medication readiness.

Education

Master’s degree

Active PA license in Indiana
