Qualifications

• DMD or DDS from CODA-accredited dentistry program.
• Active state license with a valid DEA number and CPR certification for WA and OR providers.
• Oregon providers must have a Nitrous Oxide Permit within 60 days of hire.
• Demonstrated ability to use evidence-based dentistry to assess symptoms and treatment options.
• Proven ability to coordinate, supervise, or manage others to achieve goals.
• Preferred 1–3 years of practice or completion of GPR/AEGD training.
