Qualifications

• Active State License in good standing.
• DEA license, CPR and BLS certifications.
• CSR registration required and use of Dentrix EMR.

Responsibilities

• Provide family dentistry services for pediatric patients.
• Perform prophy, extractions, fillings, sealants, crowns.
• Take x-rays and conduct exams and hygiene checks.

• Credentialing Timeframe:~1-2 days
• Requirements:Active State License, Drug Enforcement Administration (DEA), Basic Life Support (BLS), Cardiopulmonary Resuscitation (CPR), Controlled Substance Registration (CSR) and use of Dentrix Electronic Medical Record (EMR)
