  • Respond to Code Blue Alerts and perform unit inspections.
  • Minimize errors in medication use using S.T.A.R. techniques.

Job description

Evening Shift Decentralized Internal Medicine Pharmacist

The Internal Medicine Pharmacist provides pharmaceutical care including computer order entry, drug distribution, evaluation of medication orders for appropriateness of drug, dose, frequency and adequacy of monitoring, evaluation of drug interactions and allergies, and therapeutic monitoring of drugs to patients across our adult internal medicine floors.

What you will do in this role
  • Coordinate drug distribution activities in areas under supervision. Includes supervision of pharmacy technicians, order verification of medication orders, resolution of medication‑related problems and preparation of parenteral products.
  • Perform pharmacy clinical activities including providing pharmacokinetic consults, monitoring targeted drugs, antimicrobial therapy recommendations, and evaluating appropriateness of drug therapy based on disease state, host factors and cost effectiveness.
  • Serve as a source of drug information to medical and nursing staff.
  • Respond to Code Blue Alerts.
  • Perform patient care unit inspections as required to meet regulatory requirements.
  • Work to minimize errors in the medication use process by utilizing S.T.A.R. (Stop, Think, Act and Review) and other error prevention techniques, especially when dealing with a situation that is out of policy, procedure, or plan, or any other high‑risk situation. Strictly follow special handling procedures related to high‑risk, high‑alert medications and patient populations.
Qualifications
  • Clear and active Georgia pharmacist license
  • Graduate of ACPE‑accredited School of Pharmacy required; PharmD degree preferred
  • PGY‑1 residency and/or Board Certification
