Job summary

DAWN Center for Independent Living is seeking a devoted Support Coordinator for adults with developmental disabilities in northern New Jersey. The role partners with individuals and families to identify goals and access local services, focusing on independence and community participation.

The position requires travel within Morris, Sussex, and Somerset Counties, a valid driver’s license, and bilingual English/Spanish capabilities. Competitive salary and comprehensive benefits are offered.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
  • Minimum of 1 year of professional experience supporting individuals with disabilities or related populations.
  • Ability to speak proficiently in both Spanish and English to offer comprehensive support to consumers and their families.
  • Understanding of and commitment to the Independent Living Philosophy.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Experience collaborating with community agencies and service systems.
  • Comfort using Microsoft Office and other technology platforms for daily work activities.
  • Ability to work both independently and collaboratively as part of a team.
  • Valid driver’s license and reliable transportation.
  • Lived experience with disabilities is valued.

Responsibilities

  • Develop and maintain person-centered service plans based on each individual’s goals, strengths, and preferences.
  • Connect individuals and families with community-based services, supports, and resources.
  • Monitor services through monthly calls and quarterly visits and adjust plans as needs change.
  • Assist individuals in understanding and managing DDD-funded budgets and services.
  • Advocate alongside individuals and families to promote informed choice and independence.
  • Collaborate with community agencies, providers, and statewide partners including DDD, DHS, fiscal intermediaries, and other support systems.
  • Maintain timely documentation and compliance with DDD standards, policies, and deadlines.
  • Build and maintain strong working relationships with consumers, families, providers, and community partners.
  • Utilize technology and electronic systems to manage documentation, communication, and scheduling.

We are seeking a compassionate, motivated, and organized professional to join our team as a Support Coordinator serving adults with developmental disabilities in northern New Jersey.
Position Overview

The Support Coordinator partners with adults age 21+ and their families to identify strengths, goals, interests, talents, and support needs while helping them access meaningful services and opportunities within their communities.
This role combines advocacy, care management, problem-solving, and relationship-building to help individuals live more independently and achieve their personal goals. Successful candidates are adaptable, collaborative, detail-oriented, and passionate about person-centered services.
This position serves individuals in Morris, Sussex, and Somerset Counties and requires regular local travel. Reliable transportation is required.
