Job summary

Partners For Community in Manchester, CT seeks a Day Support Specialist to provide individuals with opportunities to develop valued skills and relationships. The role requires initiative and following through with program responsibilities to maximize participation.

You will support daily tasks, supervise activities, transport clients safely, document plans and progress, and maintain records. A high school diploma, valid driver's license, DDS familiarity, and timely certifications are required

Qualifications

High School diploma or GED.
Valid driver’s license with reliable transportation.
Knowledge of DDS systems.
Medication Certification through DDS, First Aid, CPR and PMT certifications must be acquired within six months of hire date and maintained throughout employment.

Responsibilities

Performance of daily program tasks to actively engage individuals.
Teach, assist and supervise individuals in activities/jobs relative to program scope.
Transport individuals to, from and between activities and work sites.
Maintain on-going case records of individuals needs and services.
Document ISP objectives, behavior data, participation and production for each assigned individual.
Notify supervisor/team of any behavioral or routine changes.
Any other duties as assigned by the Program Coordinator.
