Qualifications

Responsibilities

• Assess, monitor, and document residents’ wounds and skin conditions.
• Develop and carry out wound care plans with residents, families, providers and the interdisciplinary team.
• Perform wound treatments and monitor residents’ response to care.
• Identify and address risks for skin breakdown.
• Educate and support staff on wound prevention and treatment practices.
• Maintain accurate clinical documentation and follow care standards.
