Job summary

Roscoe Rehabilitation and Nursing Center in Roscoe, NY is seeking a compassionate Social Worker to support residents and families through the care journey. You will assess psychosocial needs, coordinate services, and help plan care to improve quality of life in a resident-centered environment.

Responsibilities include completing psychosocial assessments, facilitating care plan meetings with the team and families, providing counseling, coordinating discharge planning, and documenting interactions

Qualifications

• Bachelor's Degree in Social Work required
• Minimum of one year of experience working with a geriatric population in a healthcare setting
• Experience with MDS and discharge planning preferred
• Strong communication, organizational, and problem-solving skills
• Ability to build positive relationships with residents, families, and interdisciplinary team members
• Commitment to providing compassionate, resident-centered care

Responsibilities

• Complete psychosocial assessments and social histories upon admission
• Assist residents and families with advance directives, care planning, and transition support
• Facilitate care plan meetings with residents, families, and the interdisciplinary team
• Provide counseling, support, and interventions related to emotional, social, and behavioral needs
• Coordinate discharge planning and referrals to community resources and home care services
• Complete MDS assessments and documentation as assigned
• Document resident and family interactions in the electronic medical record
• Serve as a liaison between residents, families, staff, and outside agencies
• Participate in QAPI initiatives and contribute to quality improvement efforts
