As an LPN, you will administer medications, monitor residents, document care in EMR, and collaborate with RNs, CNAs, and physicians to maintain high-quality outcomes for our residents.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, active Missouri Licensed Practical Nurse (LPN) license in good standing.
• Current BLS/CPR Certification (or ability to obtain).
• Previous experience in skilled nursing, long-term care, rehabilitation, post-acute care, assisted living, or hospital nursing is preferred but not required.
• Strong clinical, communication, and organizational skills.
• Ability to work effectively as part of an interdisciplinary healthcare team.
• Passion for providing exceptional, resident-centered care.

Responsabilités

• Provide compassionate, high-quality nursing care to residents in a skilled nursing and rehabilitation setting.
• Administer medications, treatments, and physician-ordered interventions within your scope of practice.
• Monitor resident conditions and promptly report changes to the Registered Nurse (RN) or physician.
• Perform nursing assessments and assist with developing and updating individualized care plans.
• Accurately document resident care in the electronic medical record (EMR).
• Collaborate with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), physicians, and the interdisciplinary healthcare team.
• Supervise and support Certified Nursing Assistants (CNAs).
• Promote resident comfort, dignity, safety, and quality of life.
• Ensure compliance with all federal, state, and facility policies and regulations.
• Participate in infection prevention and quality improvement initiatives.
• Assist with admissions, transfers, and discharges as needed.
