Qualifications

• Registered nurse with current Pennsylvania licensure.
• Experience in palliative/hospice care preferred.
• Self-directed with ability to work with minimal supervision.

Responsibilities

• conducts comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using standard nursing processes.
• Develops care plans based on nursing diagnoses and palliative actions.
• Counsels patients and families to meet nursing and related needs.
• Maintains patient comfort and quality of life and coordinates with the care team.
