Northwest Kidney Centers seeks a full-time day shift Home Program Nurse Care Manager (Home Dialysis Training Nurse) to support the Kirkland Kidney Center as well as Bellevue and Snoqualmie Ridge Kidney Centers in peritoneal dialysis.

The Home Program Nurse Care Manager is responsible for delivering and documenting care for patients dialyzing at NKC and in their home, using a collaborative model to improve outcomes and quality of care, including assessments, planning, education, training, and

Qualifications

• Equivalent of Associate or Baccalaureate degree with nursing major.
• Current registered nursing license in the state of Washington.
• One year of previous nursing experience.
• American Heart Association BLS

Responsibilities

• Deliver and document patient care for home dialysis.
• Assess, plan, direct, and evaluate care for patients and their home environment.
• Educate and train patients and families on home dialysis.
