Qualifications

Responsibilities

• Assist with activities of daily living (ADLs) including bathing, dressing, grooming, toileting and personal hygiene.
• Provide companionship and emotional support.
• Assist with mobility, transfers and fall prevention.
• Prepare meals and assist with feeding when needed.
• Perform light housekeeping, laundry and household tasks.
• Provide medication reminders as permitted by policy.
• Assist with errands, transportation to appointments when authorized.
• Observe and report changes in clients’ conditions.
• Maintain documentation and communicate with care management team.
• Deliver compassionate care while preserving dignity, privacy and safety.

Job Responsibilities

• Provide compassionate care while maintaining client dignity, privacy, and safety.

Qualifications & Requirements

• Previous caregiving, home health, CNA, or healthcare experience preferred.
• Must be dependable, punctual, and professional.
• Must have reliable transportation.
• Must have a working cell phone and be able to communicate with the office.
• Must be comfortable assisting clients with personal care.
• Ability to follow individualized care plans and company policies.
• Must successfully complete required background checks and onboarding requirements.
• Experience with dementia, hospice, Hoyer lifts, and mobility assistance is a plus.
• Strong communication skills and a positive attitude.
