Job description

Day Services - Direct Support Professional II

Direct Care Service Providers (e.g., Non-licensed; registered; certified)

Provide skill building and support functions for persons with developmental disabilities to live, learn, and work successfully as an active part of their communities. Responsibilities include provision of skill building and support in areas of personal care, socialization, community integration/ participation, employability, health and safety, and other adaptive living skills.

ESSENTIAL FUNCTIONS:

  • Provide skill building and support to individuals served to promote maximum independence and facilitate participation in community and/or facility-based activities in accordance with the individual’s expressed preferences and choices. As outlined in the individual’s person-centered plan, provide training, skill building and support to include personal care, socialization, community integration/participation, volunteer opportunities, and other daily living skills. Services may be delivered during the day, evenings, weekends, and holidays.
  • Responsible for accurate and informative documentation reflecting services and supports delivered to individuals served in accordance with their person-centered plan and expressed activities of choice. Complete daily documentation of progress notes, medication administration, time sheets, and other reports in a timely and professional manner.
  • Effectively intervene in crises and emergencies, to include administration of first aid, following agency policies and procedures and seeking consultation from supervisor and other resources as appropriate.
  • Ensure all required trainings are completed in compliance with agency policies and state and federal regulatory bodies including, but not limited to, DBHDS, DMAS, and HCBS requirements.
  • Provide input into the development of person-centered plans for support that reflect the individual’s strengths and preferences.
  • Administer and document medications to individuals served in accordance with physician’s orders and established procedures.
  • As assigned, provide job skills training as outlined in individual’s Plan for Support to include hands‑on instruction, job set‑up, maintenance of materials and supplies at work stations, and quality control.
  • Transport individuals using Agency vehicles to and from community‑based activities.

OTHER DUTIES:

  • Participate in meetings held for a variety of purposes including reviews for persons served and other assigned meetings.
  • Participate in the agency’s accident‑prevention program and actively promote safe practices in the delivery of services and manufacturing of products.

QUALIFICATIONS:

  • Knowledge of developmental disabilities and related conditions
  • Ability to project a professional image while interfacing with the community.
  • Ability to work independently and as part of a team. Must be able to work without direct supervision.
  • Good oral and written communication skills
  • Basic computer skills to include keyboarding.
  • Ability to carry out supports with daily living skills to include feeding, personal care, etc.
  • Ability to safely operate Agency passenger vehicles including a variety of automobiles, vans, and buses.
  • Willing to work flexible hours including evenings, weekends, and holidays.

One‑year successful experience working with persons with developmental disabilities preferred.

High School diploma or equivalent required

Valid driver’s license and copy of DMV safe driving record.

Completion of medication aide training within five (5) months of hire

Criminal background and DSS registry check required.
