Overview

Assist with adults with I/DD disability in a day program setting. Implement individual Person Centered Plans that promote independence while instructing, modeling, and assisting the consumer in daily living skills, socialization, leisure and/or recreational activities, educational and vocational skills.

  • Position Title: Day Supports Worker
  • Setting: Individual (T2021) or Group (T202HQ)
  • Role Type: Paraprofessional &/or Associate Professional (NC Innovations Waiver/Cardinal Innovations)
Responsibilities
  • Receive, read, and be familiar with the Employee Handbook, personnel policies, and procedures for the agency.
  • Demonstrate knowledge of the basic principles of I/DD and the ability to work with persons who may differ in ability, age, economic status, or social background.
  • Show empathy, understand consumer problems, and respond appropriately.
  • Determine consumer needs and engage, building rapport with the person served.
  • Become familiar with and implement the individual’s habilitation plan such as an Individual Support Plan or other identified goals according to the service definition.
  • Document the service provided in progress notes. Include observations, consumer participation, progress, behavior changes, and any pertinent information. All documentation must be completed on the day the service is provided and in black ink only; use of white‑out is not acceptable.
  • Adhere to company policy that prohibits taking a supported person to a staff member’s private residence or leaving a consumer unattended in a vehicle or at any other public place.
  • Submit all required documentation by the deadlines specified by the Supervisor.
  • Obtain and keep current all training required by the agency, according to the service provided and the individual’s needs. Failure to renew training by the deadline results in immediate removal from the schedule until training is up‑to‑date.
  • Attend all mandatory staff meetings and consumer conferences. If unable to attend, give advance notice to the Supervisor.
  • Receive supervision from a Supervisor/Qualified Professional (QP) or Associate Professional (AP) at least quarterly and as needed, either face‑to‑face, via phone, or through group staff meetings.
  • All paraprofessionals and associate professionals will be supervised by a Qualified Professional in accordance with G.S. NCAC 27 G .0204(b)(c)(f) and the licensure or certification requirements of the appropriate discipline.
  • Work only authorized hours unless specifically approved in advance by the Supervisor/Administrator.
  • Report changes in a person’s condition, family situation, or needs to the Supervisor.
  • Report any allegation of consumer abuse, neglect, or exploitation to the Supervisor so it may be investigated, and complete an incident report along with a report to the local DSS.
  • Report any destruction or theft of agency property to the Supervisor.
  • Report any possible fraud against the consumer or agency to the Supervisor for investigation.
  • Perform other duties as required by circumstances.
Qualifications
  • Must be at least 25 years old.
  • Must have a High School Diploma or GED.
  • Must be able to read, write, understand, and follow directions.
  • Must have no substantiated findings on the NC Health Care Abuse Registry.
  • Must meet SBI and DMV requirements of LME/MCO contracts.
  • Must have a valid driver’s license.
  • Must have 3 full years of driving experience.
  • Must have an acceptable level of automobile liability insurance.
  • Experience with persons with intellectual/developmental disabilities preferred but not required.
  • Must be willing to complete all required trainings as assigned.
  • Must have access to a device with an internet connection at the time services are rendered.
