Job summary

Community-Residences in Winsted, CT is seeking a Day Program Instructor to support clients in daily living, social and physical activities. This full-time position requires active mobility and ability to follow Individual Plan of Care.

You will document progress, assist with meals, transport when needed, maintain safety and privacy, and participate in trainings. A valid CT driver’s license is required, and a high standard of communication is essential.

Qualifications

  • Bachelor's or Associate degree preferred; 1 year experience preferred or required; high school diploma or GED minimum.
  • Valid CT Driver’s License; ability to drive agency vehicle; maintain good driving record.
  • Ability to lift and transfer up to 70-75 lbs and work actively inside/outside the facility.

Responsibilities

  • Develop strategies to help clients cope with social, emotional, and physical daily living.
  • Implement Individual Plan of Care and document progress, needs, barriers.
  • Monitor medical, physical, and behavioral changes; report within 24 hours to supervisor.
  • Assist with meals per dietary plans; follow dietary requirements.
  • Maintain appearance, cleanliness, and safety of day program, properties and vehicle.
  • Maintain clients' privacy and adhere to confidentiality policy.
  • Participate in trainings, meetings, and required trainings.
  • Chaperone community outings, ensuring health and safety in line with plans.
  • Handle petty cash and client funds; keep records accurate.
  • Familiarize with CRI policies; ensure compliance.
  • Perform other duties as assigned.

ESSENTIAL JOB FUNCTIONS



  • 1. Physical Demands: must be able to lift and transfer 70-75 lbs. per Day Program Instructor. Work is active and performed inside and outside of the facility.

  • 2. Must be able to utilize approved protective hold techniques in order to keep you, clients, and co-workers safe.

  • 3. Create a learning environment; develop strategies to help clients cope effectively with social, emotional, and physical situations in daily living, and facilitate independent performance in these areas.

  • 4. Implement Individual Plan of Care/Follow-Along Plan based in the Interdisciplinary Team’s identification of client needs; record data on all identified teaching strategies, behavior plans, report client’s progress needs and any barriers that impede progress. Responsible to maintain and document an awareness of individual treatment plans, inclusive of use of leisure time, community experiences, use of public transportation, safety skills, personal hygiene and activities of daily living.

  • 5. Monitor medical, physical and behavioral changes in individuals on a daily basis and report changes to the Supervisor, R.N. Program Coordinator or Program Director as it occurs, is observed or discovered; write up incident reports and submit to Program Coordinator or Day Program Supervisor within 24 hours.

  • 6. Follow written dietary plans for each client; assist with meal preparation, if required.

  • 7. Maintain the appearance, cleanliness and upkeep of the day program, properties and vehicle, insuring they are clean and safe. Report all repair needs to the Program Supervisor and/or Program Coordinator in a timely manner.

  • 8. Maintain clients' right to privacy, and adhere to CRI's Confidentiality Policy.

  • 9. Conduct self in a professional manner when interacting with participants, supervisors, co-workers, families, medical profession and members of the community.

  • 10. Participate in training opportunities (in-service training, staff meetings, conferences, etc.), as required; maintain all required training per agency policy and DDS regulatory requirement.

  • 11. Take individuals to a variety of community based outings ensuring that all health and safety measures in the Individuals support plan are followed while in the community. Utilizes the agency vehicle following state laws and safety procedures.

  • 12. Provide accurate accounting of agency’s petty cash and client funds in an informative, auditable condition at all times, as required or requested by the Program Supervisor.

  • 13. Familiarize oneself with CRI policies and procedures, ensuring compliance with same.

  • 14. Perform other work related duties as assigned by the immediate supervisor and/or other management as required.

  • 1. Possess good to excellent oral and written communication skills.

  • 2. Demonstrate thoroughness and follow-through on daily work assignments.

  • 3. Display motivation and interest in improving quality of life for population served.

  • 4. 4. Demonstrate and maintain an optimistic attitude, patience, and compassion for and with the population served.

  • 5. 5. Valid CT. Driver’s License, ability to drive own and agency vehicle and a good driving record. Public Service License may be required depending upon type of transport vehicle used; Must be obtained within sixty (60) days of hire or transfer; PSL must be maintained for the duration of employment at day program.


EDUCATION:



  • A Bachelor's degree in related Human Services area or an Associate's degree in a related field and one (1) year experience is preferred. Minimum requirement for employment is a high school diploma or GED.
