Job summary

Open Sky Community Services, in central Massachusetts, seeks a Day Hab Support Staff to provide direct-care support to adults with intellectual disabilities within our Day Habilitation program. You will implement individualized service plans, promote community involvement, ensure safety, and collaborate with an interdisciplinary team to help participants live more independently.

Open Sky's Day Habilitation programs serves adults between the ages of 23-85 with a primary diagnosis of intellectual disabilities, though some individuals may also have mental health diagnoses and/or physical challenges. Most participants require supportive structures and staff assistance to maximize their involvement in activities within the program as well as within the local community. Our staff educate through the creation and implementation of individualized service plans, daily curriculum/activities, and have a heavy focus on community integration.

Open Sky's Day Habilitation programs serves adults between the ages of 23-85 with a primary diagnosis of intellectual disabilities, though some individuals may also have mental health diagnoses and/or physical challenges. Most participants require supportive structures and staff assistance to maximize their involvement in activities within the program as well as within the local community. Our staff educate through the creation and implementation of individualized service plans, daily curriculum/activities, and have a heavy focus on community integration. As a Day Hab Support Staff, you will provide support to the individuals served in a direct care role by ensuring a supportive and structured environment. They also provide guidance, advocacy, and life skills education for individuals served within the Day Program.
