Job summary

Vinfen in Boston, MA is seeking a Day Hab Program Activity Assistant (Instructor) to help implement clinical and individual programs and daily activity schedules for persons served.

You will provide direct support, assist in case management, maintain documentation, and contribute to team reviews, with a focus on engagement, skill-building, safety and community integration.

Qualifications

• Experience applying Positive Behavior Support (PBS) and Safety Care in program settings.
• Knowledge of applied behavior analysis with developmental disabilities population.

Responsibilities

• Provide instruction and skill building in Self Help, Sensorimotor, Communication, Social Development, Independent living, Behavior Development, Affective Development, and Wellness Development.
• Assist in case management responsibilities, as directed.
• Provide input for interdisciplinary team reviews.
• Assist in development of DHSP for persons served, including recommendations from professional and para-professional specialists.
• Maintains documentation daily and progress notes per guidelines.
• Works directly with persons served individually and in groups to develop skills.
• Ensure meaningful activities and daily engagement with choices where possible.
• Communicate with management regarding treatment and programs for persons served.
• Participate or be involved with teams such as Case Management Team.
• Support members in crisis management using PBS and Safety Care.
• Assist with training of staff as needed.
• Maintain physical upkeep of the program area; monitor supplies and equipment; notify management of needs.
• Act as advocate for persons served and promote community integration.

Job description

The Day Hab Program Activity Assistant (Instructor) aids in the implementation of clinical and individual programs. Assists in the daily implementation of DHSP, daily schedule of activities, and recommended plans. Provides support to persons served in day habilitation setting. Assists in case management responsibilities.

Responsibilities

• Provides instruction and skill building in the areas of Self Help, Sensorimotor, Communication, Social Development, Independent living, Behavior Development, Affective Development, and Wellness Development.
• Assists in case management responsibilities, as directed.
• Provides input for interdisciplinary team reviews.
• Assists in the development of DHSP for persons served, including recommendations from professional and para-professional specialists. Maintains contact with family members, funding source, and other staff members, including members of the Interdisciplinary team at the Day Hab.
• Implements clinical and daily plans for persons served. Provides recommendations or implements changes to ensure continuous improvement of programs.
• Maintains documentation daily. Adheres to data collection and maintenance policies and guidelines. Ensures progress notes for persons served are completed according to guidelines.
• Works directly with persons served individually and in groups. Works to develop skills for persons served.
• Ensures that members are actively engaged in meaningful activities throughout the entire day, offering choices wherever possible.
• Communicates with management regarding treatment and programs for persons served.
• Participates or is actively involved with various teams, such as Case Management Team.
• Supports members in crisis management using Positive Behavior Support (PBS) and Safety Care.
• Assists with the training of staff, as needed.
• Maintains physical upkeep of the program area. Monitors supplies and equipment needed. Ensures equipment is used, cleaned, and stored appropriately. Notifies managers of any needs identified and works to resolve those
• Acts as advocate for persons served in all areas. Follows policies and procedures specific to program and Vinfen. Actively works toward community integration of persons served through community opportunities, volunteer opportunities, reverse integration, and the development of friendships.

Qualifications

Typical Requirements:

One year of relevant work experience/training.

Preferred /Required Education

A high school diploma or equivalent is required.

Driving Requirements

Driving is a requirement for this position using either a Vinfen van or personal vehicle. If using a personal vehicle, you must possess and maintain adequate insurance as well as maintain a safe driving record which is subject to annual checks. A valid driver's license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver's license for at least six months and must be able to pass a driver's screening background check.

Physical Effort

Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position.

Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift.

Ability to operate a computer and other office equipment such as a calculator, copier, and printer.

Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways.

Ability to assist clients with tasks of daily living.

Ability to speak, hear, and communicate with clients, staff, and external representatives.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Pay Range
