Job summary

Prime Home DDS is seeking a Day Assistant Coordinator / Health Care Coordinator to support individuals with developmental disabilities through healthcare services, medication oversight, staff training, and regulatory compliance. The role collaborates with day and residential services, medical professionals, families, and support staff to ensure quality outcomes.

  • Coordinate communication with nursing staff regarding PRN medication administration.
  • Ensure medical orders are processed promptly and staff trained on medical care.

Job description

Prime Home DDS (PHDDS) is seeking a dedicated and compassionate Day Assistant Coordinator / Health Care Coordinator to support individuals with developmental disabilities by ensuring high-quality healthcare services, medication management, staff training, and regulatory compliance.

The successful candidate understands that enhancing both a person's image and competency are fundamental to creating, supporting, and defending valued lives. This role works closely with day services, residential services, medical professionals, families, and support staff to ensure quality outcomes for the individuals we serve.

Qualifications
Required Certifications
  • Medication Aide Certification
  • CPR Certification
  • First Aid Certification
  • Mandt Training (or ability to obtain)
Required Knowledge and Experience
  • Knowledge of medical management and medication administration within the developmental disabilities and mental health fields.
  • Understanding of federal, state, and local regulations governing community-based services.
  • Knowledge of best practices in developmental disability services.
  • Experience coordinating healthcare services and medications.
  • Ability to work effectively within a multidisciplinary team.

Essential Duties and Responsibilities
Leadership
  • Model and promote Prime Home DDS's mission, values, and principles.
  • Motivate and support staff and individuals receiving services to achieve personal and organizational goals.

  • Identify and mentor future leaders within the organization.
  • Participate in employee orientation and ongoing staff development.
  • Conduct home visits as directed by management.
  • Attend referral meetings and assist with residential and day service census growth.
Healthcare and Medication Oversight
  • Ensure all medical procedures beyond the scope of a Medication Aide are properly trained, competency-tested, and monitored according to approved medical protocols.
  • Review and oversee all PRN (as-needed) medication protocols.
  • Coordinate communication with nursing staff regarding PRN medication administration.
  • Ensure Medication Administration Records (MARs) contain active and accurate physician orders.
  • Participate in medication delivery, verification, and acceptance processes.
  • Attend intake meetings for individuals requiring specialized medical monitoring.
Compliance and Quality Assurance
  • Monitor compliance with Medication Aide regulations and agency policies.
  • Conduct quality assurance reviews of Medication Administration Records (MARs).
  • Ensure medical documentation is complete, accurate, and up to date.
  • Review incident reports, injuries, and medical concerns as required.
Team Collaboration
  • Attend weekly meetings with Day Service Coordinators and Residential Coordinators.
  • Participate in ISP, IPP, IEP, and other planning meetings as necessary.
  • Collaborate with healthcare providers, families, support teams, and agency leadership to ensure quality care.
Communication
  • Maintain timely communication with Residential Coordinators, Day Service Coordinators, nursing personnel, and other stakeholders regarding medical care and service needs.
  • Ensure medical orders are processed promptly.
  • Coordinate and communicate staff training requirements related to medical care and medication administration.
Training
  • Provide on-site training as needed within day service locations.
  • Monitor staff competency and compliance with healthcare-related procedures.
Additional Responsibilities
  • Collaborate with Behavior Support teams as needed.
  • Perform lead staff responsibilities as assigned.
  • Complete additional duties as required to meet the needs of Prime Home DDS.
