Qualifications

• DPT from an accredited program.
• Active PT license in Texas.
• Minimum 1 year of home health experience.

Responsabilités

• Provide skilled physical therapy services in the home health setting.
• Complete OASIS assessments and patient visits.
• Develop and implement individualized treatment plans to improve mobility and function.
• Collaborate with physicians, nurses, and care team to coordinate care.
• Maintain accurate clinical documentation and meet productivity expectations.
