The ICM Case Manager provides a comprehensive range of paraprofessional social services to patients, clients, and their families and will work across both the Bibb County and Baldwin County programs. This role actively participates in treatment team planning, delivers crisis intervention when needed, and assists individuals in addressing personal and social challenges. Responsibilities include providing supportive counseling, serving as a liaison for social services, coordinating case management activities, and facilitating group skills training to promote the overall well-being of individuals in the program.
Schedule: Monday-Friday 8:30am-5:00pm
