Qualifications

Responsibilities

• Perform plan of care interventions including medication administration and procedures.
• Monitor patient condition and communicate with care team, physicians, patients and families.
• Coordinate care across disciplines including transfers, discharge and referrals.
• Provide patient/family education appropriate to age, culture and condition.
• Advocate for the physical and emotional well‑being of the patient.
