Job summary

Southeast Health seeks a Clinical Transition Coach (CTC) to facilitate safe patient transitions from acute care to home or facility-based care. The role supports discharge planning, admissions, and throughput as part of a healthcare team in Alabama.

The LPN license is required with at least 3 years in medical-surgical nursing, and the candidate must communicate effectively in English, delivering strong customer service and collaborative leadership in a fast-paced setting.

Qualifications

• Current (unencumbered) LPN license in Alabama.
• Minimum of 3 years nursing experience in a medical-surgical setting.

Responsibilities

• Assist in discharge planning and patient throughput.
• Coordinate transitions to home care or skilled nursing facilities as appropriate.
• Assist with admissions processes when needed.

Always the right career direction.Job Description SummaryThe Clinical Transition Coach (CTC) assists in the safe and timely transition of patients from acute inpatient care to alternative levels of care, including but not limited to a home care program, palliative/hospice program, skilled nursing facility, or home setting appropriate to the patient’s needs.
