Job summary

Jefferson Health is seeking a professional nurse to deliver patient- and family-centered care using the nursing process. The role emphasizes safety, cultural beliefs, and collaboration with designated care partners across Bucks and surrounding areas.

The Nurse will develop individualized care plans, educate patients and families, and participate in safety and quality improvement initiatives while working nights in a full-time capacity.

Qualifications

  • Graduate of an accredited school of nursing.
  • Knowledge of patient- and family-centered practice.
  • Cultural awareness and ability to collaborate with diverse patients and families.
  • Effective communication concepts and techniques in partnerships with patients and families.
  • Knowledge of patient assessment and care planning.
  • Education for patients and families on condition, treatment, and recovery steps.
  • Medication administration and prescription knowledge for post-discharge care.
  • Discharge planning and follow-up care coordination.
  • Use of health information documentation methods.
  • Basic Life Support (BLS) certification.

Responsibilities

  • Interacts with co-workers, patients, families, and staff in line with Jefferson Health values.
  • Integrates evidence-based practice and clinical expertise with patient preferences and cultural beliefs.
  • Partners with patients and families to identify learning needs and provide education.
  • Minimizes risk by following nursing practices and protocols.

  • Monitors care process outcomes and participates in quality improvement initiatives.
  • Collaborates with inter-professional teams to ensure safe, coordinated care.
  • Recognizes patient preferences and cultural beliefs in care delivery.
  • Demonstrates problem solving and conflict resolution to foster belonging.
  • Pursues ongoing professional growth and acts as a primary preceptor when appropriate.
  • Participates in hospital and practice committees focused on safety and quality.
