Qualifications

• Registered nurse (RN) licensed or eligible in the state of practice.
• Graduate of an accredited nursing program.
• Current BLS certification required.
• Additional certifications may be required by area of practice.

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational and discharge planning needs.
• Formulate and implement a plan of care including discharge planning.
• Provide direct patient care, coordinate and supervise care by the health care team.
• Evaluate patient progress and adjust the care plan as needed.
• Document assessments, plans, and responses in the patient record.
• Participate in evidence-based practice and quality improvement activities.
• Support growth and education of facility personnel and students.
