Job summary

Community Health Resources (CHR) is seeking an Outreach Therapist in Manchester, CT for the Mobile Crisis Intervention Services program. This full-time position offers a flexible schedule (Mon-Fri, 10am-6pm) with supervision and a supportive team environment, plus a company car and quarterly productivity bonuses.

You will assess and treat clients, document progress, and develop discharge plans while collaborating with families and other providers to support children and families in crisis.

Qualifications

• Master’s degree or higher in mental health field.
• LCSW, LMFT, or LPC eligible; Licensed preferred.
• Demonstrated experience as a psychotherapist working with mental illness or serious emotional disorders.
• Experience in assessment, evaluation and provision of individual, group and family therapy.

Responsibilities

• Provide assessment and treatment interventions aligned with evidence-based models.
• Document client progress and outcomes.
• Develop discharge plans with natural supports.
• Maintain collaborative relationships with family and service providers.
• Provide crisis assessment and intervention during client crises.
• Perform home and community-based care.
• Maintain client and staff records per agency policy.
• Cover for team members during absences; participate in supervision.
• Transport clients using agency or personal vehicle.

EMPS delivers 24/7 crisis support for children and families experiencing emotional, behavioral, or situational distress. Services include:

• Crisis telephone triage and support
• Mobile crisis assessment and intervention
• Community-based care in homes, schools, courts, and other settings

Our mission is to provide early intervention, stabilization, and alternatives to hospitalization while strengthening families and communities.

Duties & Responsibilities

• Provides assessment and treatment interventions consistent with fidelity to evidence based model if applicable.
• Documents ongoing client progress and outcomes
• Demonstrates competency in age related or specialty issues and developmental needs for each population served.
• Works with clients to develop appropriate discharge plans utilizing natural supports.
• Maintains collaborative relationships with family members and other service providers
• Provides crisis assessment and intervention to monitor status of clients in crisis.
• Performs home and community based care.
• Responsible for Maintaining Client and Staff Records According To Agency Policy
• Provides Coverage for team members during periods of absence.Participates in weekly supervision and meetings to review client and program progress.
• Transports clients, when necessary using agency vehicles and/or personal vehicle
• Additional duties as required.

QUALIFICATIONS :

Education: Master’s Degree or higher in mental health field

Experience: Demonstrated in-depth knowledge and experience as a psychotherapist working with people with mental illness and/or serious emotional disorders. Demonstrated training and experience in all aspects of mental health treatment, assessment, evaluation and provision of individual, group and family therapy. Relevant experience to program model to which individual is assigned.

Licensure/Certification/Registration: LCSW, LMFT, LPC, eligible. Licensed preferred

*Actual rates within this range are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. (Please note, for part-time positions this range would be pro-rated based on hours worked)
