UnityPoint Health is seeking a Crisis Services Case Manager in the Des Moines region to provide client-centered case management and maintain comprehensive records. This community-based role collaborates with mental health practitioners, law enforcement, schools, and hospitals to connect clients with crisis services.

The position supports scheduled MCRT daytime on-call coverage, with a Monday–Friday 8:00 a.m.–4:00 p.m. shift.

Qualificações

• Mental health professional as defined in Iowa Code section 228.1.
• Bachelor's degree; or Associate's degree; or High school diploma.
• Mental health professional: 1 year of mental health experience
• Bachelor's in human services: 1 year
• Bachelor's in non-human services: 2 years
• Associate's degree: 2 years
• High school diploma: 2 years
• Valid driver's license and compliance with Eyerly Ball's auto insurance policy
• Must pass criminal records and dependent adult/child abuse checks

Responsabilidades

Meet virtually with our team and learn how you can support individuals experiencing mental health crises in their communities.

Employment option: Full-time, day shift

Current openings: Des Moines, Iowa. This is a community-based role that includes client outreach and scheduled MCRT daytime on-call coverage.

What You Will Do

Provide case management services to clients, maintain accurate client files and documentation, build working relationships with mental health practitioners, law enforcement, schools, hospitals, and community partners, share information about available crisis services through outreach and presentations, and cover scheduled MCRT day shifts.
