Responsibilities

• Complete an initial screening and/or assessment within 24 hours of arrival on the crisis unit.
• Assist residents with case management needs including housing, social support, and resource coordination.
• Coordinate care between deflection/crisis respite and mental health outpatient and aftercare providers.
• Complete intake documentation with clients including financial information.
• Prepare residents for discharge and remove barriers to success.
• Create person-centered treatment plans with goals and strategies.
• Assist in direct care on the unit and observe residents.
• Facilitate psychoeducational groups daily and document activities.
• Provide therapeutic intervention in crisis situations and ensure appropriate care.
