Qualifications

Responsibilities

• Credentialing, re-credentialing and privileging process for all healthcare providers to provide patient care at the Hospital.
• Ensure data for each provider in credentialing databases and online systems; timely renewal of licenses and certifications.
• Complies and maintains current and accurate data for all providers.
• Completes provider credentialing and re-credentialing applications; monitoring and follow-ups as needed.
• Ensure practice addresses are current with all entities.
• Maintain copies of current licenses, DEA certificates, malpractice coverage and other required documentation.
• Perform other duties assigned by coordinator.
