Job summary

SMA Healthcare in Palatka, FL is seeking a mental health professional to conduct psychotherapy and coordinate individualized treatment plans for clients with complex psychiatric and substance use concerns.

The role requires assessing diagnostic history, planning treatment, and collaborating with a multidisciplinary team to ensure optimal client outcomes. Strong documentation and communication skills are essential.

Qualifications

  • Bachelor's degree in Psychology, Social Work, Counseling or related Human Services discipline required.
  • Masters degree preferred; licensure/certification as Addictions Professional or Clinical Social Worker, Mental Health Counselor, or Marriage and Family Therapist preferred.
  • Experience with the target population may be required per program guidelines.

Responsibilities

  • Conducts psychotherapy (individual, group, and/or family) as indicated.
  • Assesses psychiatric history, mental status, diagnosis, and risk behaviors.
  • Develops and updates an individual treatment plan.
  • Monitors client progress and adjusts treatment plans as needed.
  • Educates and supports clients and families, advocating for client rights.
  • Documents progress in SMA's EMR accurately and timely.
  • Collaborates with multidisciplinary team and outside services to ensure outcomes.

Job description

Essential Job Functions:
  • Conducts individual, group, and/or familypsychotherapy as indicated by diagnosis and individualized treatmentplan.
  • Conducts comprehensive assessment ofpsychiatric history (e.g., onset, course and effect of illness, pasttreatment and responses, and risk behaviors), mental status, anddiagnosis; physical health and dental health; use of drugs or alcohol;education and employment; social development and functioning; activitiesof daily living (e.g., self-care, living situation, nutrition, moneymanagement); and family structure and relationships.
  • Develops an individual treatment plan andensures immediate changes are made in the treatment plan as clients'needs change.
  • Provides ongoing assessment of clients'co-occurring symptoms and clients' response to treatment. Makesappropriate changes in treatment plans to ensure immediate andappropriate interventions are provided in response to changes in mentalstatus or behavior which put clients at risk (e.g., suicidality).
  • Educates and supports clients and theirfamilies, and advocates for clients' rights and preferences.
  • Documents client progress in SMA's EMR tomaintain a permanent record of client activity according to establishedmethods and procedures.
  • Participates in staff organization meetingsand treatment planning review meetings.
  • As a member of the multidisciplinary team,works in close conjunction with all team members, and othercollaborating agencies to ensure the best possible client outcomes.
  • Coordinates with outside services asappropriate to ensure that clients' needs are addressed.
  • Provides practical help and support,advocacy, coordination, side-by-side individualized support, problem solving,direct assistance, training, and supervision to help clients obtain thenecessities of daily living.
  • Performs miscellaneous job-related duties asassigned.
Qualification Requirements:

Minimum Education and/or Experience:

Bachelors degreein Psychology, Social Work, Counseling or other relevant Human Servicesdiscipline required, Masters degree preferred. Certification as an Addictions Professional or Licensedto practice as a Clinical Social Worker, Mental Health Counselor, or Marriageand Family Therapist preferred. Experiencewith the target population preferred. Additionaleducation and/or experience may be required dependent on specific programguidelines.

Knowledge/Skills/and Abilities:
  • Thorough knowledge of the most recent DSM version and the abilityto formulate a diagnosis.
  • Strong clinical knowledge and skill in both substance abuse andmental health therapy.
  • Thorough knowledge of clinical assessment process, treatmentplanning, individual, group, and family therapy.
  • Ability to quickly assess and diagnose persons for treatment andformulate realistic, measurable goals and objectives.
  • Ability to establish and maintain effective working relationshipswith other employees, medical and clinical professionals, and thegeneral public.
  • Ability to prepare clear and concise written reports.
  • Ability to handle verbally and /or physically abusive andaggressive persons and utilize aggression control techniques to ensurethe safety and well-being of the person, other persons, and employees onthe team.
  • Ability to incorporate cultural and age appropriate care into allaspects of communication and service delivery.
  • Ability to communicate effectively both orally and in writing.
  • Knowledge of insurance and Medicaid billing and services.
  • Knowledge of managed care and different payer sources.
Necessary Special Requirements:

Possession of a valid Florida driver's license, acceptable drivingrecord, and proof of personal automobile insurance if required to drive an SMAvehicle and/or use a personal vehicle for SMA business. Complete State ofFlorida mandatory background screening prior to start of employment. Complete SMA required training during thefirst six (6) months of employment and updated if required.

Physical:

Mobility and ability to bend andreach during an 8-12 hour day. Able to lift minimum 10 pounds. Visual andauditory acuity sufficient to evaluate, intervene, treat, and record clienthealth care needs. Fine motor skills for legible and accurate charting, dailycorrespondence and presentation, either manually or orally. Work enduranceability to work 8-12 hour shifts with a meal break, as possible. Routine8-12hour shifts. Hours and days off may vary. Extra hours may berequired. Work assignment locations may vary.
