Job summary

Signature Health in Ohio is seeking a Counselor to provide intensive, short-term counseling to adults and children via in-person or telehealth modalities. You will use a team-based treatment plan and contribute to Psych@Home discharge planning, with responsibilities across assessment, documentation, and coordination with care partners.

Requirements include a Bachelor's degree, an Ohio-licensed credential (LSW/LISW/LPC/LPCC/LIMFT/LMFT), and AHA BLS certification.

Qualifications

• Bachelor’s degree required.
• OH licensure (LSW/LISW/LPC/LPCC/LIMFT/LMFT) license in OH required.
• AHA Basic Life Support (BLS) certification required.
• Competency in treating mental health and/or substance use disorders.
• Familiarity with state and federal healthcare regulations.

Responsibilities

• Engage in supervision with your Supervisor to address ethical and legal aspects.
• Provide occasional in-person services as needed at a clinical site.
• Complete all productivity and documentation requirements.

SCOPE OF ROLE

Reporting to the Supervisor, Behavioral Health Services, the Counselor will provide intensive, short-term counseling services to adults and children experiencing an acute exacerbation of mental health symptoms, with or without problematic substance use. Services are provided individually and in groups in an audio-visual telehealth format. The Counselor will utilize a team-focused treatment plan approach to provide patients with exceptional therapeutic intervention services.

HOW YOU’LL SUCCEED

• Conduct individual, group, or family therapy for both adults and children.


• Complete comprehensive diagnostic assessments and regularly assess suicide risk, completing safety planning as indicated.


• Provide patients orientation to the Psych@Home program, initiating discharge planning at the start of treatment.


• Regularly coordinate with internal and external provider partners as clinically indicated.


• Maintain client charting in accordance with Medicaid, JCAHO, ODADAS, ODMH, CARF and professional standards.


• Prepare all documentation for discharge and aftercare referral.


• Regularly participate in and contribute to Psych@Home team rounds.


• Acts to understand and safeguard the welfare of others.


• Actively engage in supervision with your Supervisor; actively consult with supervisor to act upon ethical and legal aspects of the work.


• May be required to perform in-person services or participate in in-person meetings (at a clinical site) as needed and as determined by Signature Health.


• Responsible for and completes all productivity/documentation requirements.


• Participate in after-hours call to accept potential admissions


• Other duties as assigned.

Requirements

KNOWLEDGE & EXPERIENCE

• Bachelor’s Degree required.


• Valid unencumbered LSW, LISW, LPC, LPCC, LIMFT or LMFT License in the state of Ohio required.


• American Heart Association (AHA) Basic Life Support (BLS) certification required.


• Competency in the treatment of mental health and/or substance use disorders within the context of individual, group, couples and family systems therapy.


• Experience collaborating with other team members to optimize outcomes clients.


• Familiarity of state and federal healthcare regulations.


• Awareness of community and state support resources for population served.

WORKING CONDITIONS

• Work is normally performed in a typical interior/office/clinical work environment.


• While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.


• Requires periods of sitting, standing, telephone, and computer work.


• Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.


• Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.


• Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.


• Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.


• Possible exposure to blood borne pathogens while performing job duties.


• Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.


• Sufficient dexterity to operate a PC and other office equipment.
