Job summary

Catholic Charities Steuben/Livingston in Bath, NY is seeking a Residential Counselor to support residents and help run the daily operation of the facility. You will conduct intake, assist with recovery planning, and ensure safety and confidentiality while working with a collaborative team.

This full‑time role requires an NYS qualified health professional license (CASAC/CASAC-Trainee/LMSW or RN) and at least two years in substance abuse or related human services.

Qualifications

• Associates preferred.
• Two years of prior experience in substance abuse or related human service field preferred.

Responsibilities

• Performs screening, intake and orientation of new residents.
• Conducts SUD assessment and diagnostic impression as part of care planning.
• Provides individual and group counseling and education to residents.
• Maintains confidentiality and ensures safe, compliant operations.

Qualifications

Education:

Associates preferred.

Credentials:

NYS qualified health professional license required- CASAC-Trainee, CASAC, LMSW or RN.

Experience:

Two years of prior experience in substance abuse or related human service field preferred. Experience and demonstrated ability to relate effectively to persons with alcohol/substance use disorder.

General Description

The Residential Counselor functions as the primary counselor in the care of the residents and assists with the daily operation of the facility.

Essential Duties and Responsibilities

• Competes screening, Intake and orientation of new residents
• Conducts SUD Assessment; SUD Diagnostic Impression; LOC as determined by LOCADTR
• Ensures individualized recovery-oriented treatment planning with the resident and care team.
• Conducts LOC as determined by LOCADTR
• Gets involved in residents’ daily activities, observes behavior and ensures the maintenance of appropriate conduct.
• Provides for the safety and integrity of the facility at all times.
• Ensures all records are locked and medications are stored and monitored according to program policy.
• Ensures the adherence of confidentiality regulations.
• Coordinates services and communicates resident’s conditions to appropriate service providers.
• Provides individual and group counseling, education and training to residents in living skills through individual and group work.
• Communicates effectively and works cooperatively with residents, staff and community professionals.
• Participates in the development and maintenance of the therapeutic community.
• Provides crisis management as needed. Has the ability to identify symptoms of decomposition and develop strategies to address concerns with appropriate treatment providers.
• Maintains professional practices.
• Adheres to Agency policies and procedures, performs other related tasks as assigned.
• Transports residents in agency vehicles to program related activities.
• Uses the daily log and progress notes to communicate information gathered during assigned shifts.
• Participates in clinical supervision sessions.

Counseling/ Case management responsibilities

• Provides Psycho-education
• Provides care coordination and referral
• Ensure all residents receive comprehensive case management services and complete access to community services for legal, mental health, social services, vocational assessments, and misc. needs.
• Ensures all residents have a medical history recorded and a physical exam performed within 45 days of admission.
• Ensures the following: comprehensive service plans are prepared within 30 days of the development of the initial service plan, service plans are reviewed at least every 30 days thereafter and signed as required by the supervisor.
• Ensures the discharge plans provide for the specific needs of a resident and prepared in collaboration with the resident.
• Ensures progress notes are prepared at least once per week and reflect the goals and objectives of the service plan.
• Completes other duties as assigned
