The role includes developing and documenting individual program plans, monitoring resident progress, ensuring timely data entry, and coordinating release and confinement plans. A four-year degree and at least one year in human services are required, with basic PC and Office skills expected.

Qualifications

• Four-year degree in social human services, criminal justice, or behavioral science from an accredited college or university.
• One year of experience in human or community-based services.
• PC experience with Windows 2000/XP; Microsoft Office Suite focusing on Word and Excel.

Responsibilities

• Supervise and implement the agency's policies and procedures in counseling and treatment programs; maintain all applicable documentation.
• Develop, coordinate, monitor, evaluate, and document individual program planning, including goals, referrals, and counseling strategies for clients.
• Guide and document resident movement, behavior, and progress; ensure timely data entry in the FreshStart Client Management System.
• Submit resident release plans, home confinement plans, and termination letters in a timely manner.
• Communicate all relevant information to supervisory staff regarding the agency, facility, and residents.
• Provide staff duty coverage as scheduled and participate in required meetings.
• Establish and maintain liaison activities with supervising authorities, other agencies, community groups, and professional associations.
