Job Summary

The Counselor is responsible for gathering data, documenting problems, and establishing a course of treatment. Provides direct addiction treatment services to adult patients and family members.

Essential Functions
  • Provide individual and group counseling to patients assigned to the caseload.
  • Direct training groups and seminars; provide therapeutic leadership in group sessions.
  • Liaise with community agencies and law enforcement to ensure continuity of treatment and compliance with legal obligations.
  • Complete individual chemical dependency assessments of new patients, ensuring all required forms are signed and accurate.
  • Develop Individual Treatment Plans (ITP) within the established timeframe; verify patient signatures.
  • Update new patient charts within 30 days and maintain current treatment plans reflecting goals.
  • Coordinate with medical staff to obtain urine samples for monthly urinalysis and document results.
  • Participate in diversion control, including random bottle recalls.
  • Submit necessary forms to the Medical Director for take‑out medications, dose changes, phase changes, MSW schedules, and medication adjustments.
  • Ensure confidentiality by completing release of information for all outside referrals.
  • Maintain patient files in compliance with DHHS regulations, protecting them from unauthorized access.
  • Complete discharge documentation within seven days from the date of discharge.
  • Contribute to ongoing assessment and treatment planning via daily reports, case reviews, and treatment teams.
  • Provide patient education about chemical dependency and recovery.
  • Offer supportive counseling or crisis intervention with medical and nursing staff as needed.
  • Deliver public education on opioid treatment programs to the general public and professional community.
  • Participate in professional development, including internal and external in‑service programs, and maintain professional credentials.
  • Engage in Continuing Quality Improvement, collecting and reporting on assigned CQI monitors.
Essential Qualifications
  • Degree in counseling or a related field, or equivalent experience, or currently pursuing LCDC certification (internship). Must possess a valid driver's license or state‑issued ID.
Physical Requirements
  • Frequently sit at a desk or computer while typing, completing paperwork, or meeting patients.
  • Occasionally lift or carry items, paperwork, files, office supplies up to 50 lbs.
  • Occasionally push/pull exerting up to 5‑10 lbs of force; open doors, file drawers, push files.
  • Occasionally kneel/crouch and reach lower file drawers; stretch to retrieve materials.
Mental Requirements

Position requires constant mental alertness, strong attention to detail, and high accuracy. Must follow oral and written instructions, work independently, and exercise sound judgment in critical clinical decisions.
