EmpowerMe Wellness in Belleville, IL seeks licensed therapists to deliver physical, occupational, or speech therapy to older adults within the EmpowerMe 360 model. You will assess, plan, and implement evidence-based interventions as part of a collaborative care team to promote independence and reduce fall risk.

You’ll participate in care coordination, telehealth support, and continuous quality improvement while maintaining ethical standards and patient-centered care.

Qualifikationen

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Passion for empowering seniors to live their best lives.
• Strong clinical skills with a collaborative spirit.
