SYNERGY CARE, INC. in Lake Charles, LA is seeking a licensed Occupational Therapy Assistant (COTA) for a full-time float position, working across Assisted Living, Memory Care, and outpatient clinics. The role emphasizes rehabilitation, patient-centered care, and functional outcomes.

Responsibilities include providing OT treatments, following OT plans, and collaborating with an interdisciplinary rehab team while maintaining EMR documentation.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program or graduating within 6 months.
• NBCOT certified (COTA/L) and eligible for Louisiana OTA license.
• Experience in SNF, assisted living, memory care, or outpatient rehab preferred.

Responsibilities

• Provide occupational therapy treatments to residents in AL, memory care, and outpatient settings.
• Implement treatment plans established by the Occupational Therapist (OT).
• Focus on ADLs, functional mobility, cognitive therapy, and safety training.
• Collaborate with an interdisciplinary rehab team.
• Maintain accurate EMR documentation and compliance standards.
