Qualifications

• Must hold a valid state license and registration.
• Provide treatment under supervision of Occupational Therapist.
• Experience in long-term/geriatric care preferred.
• Strong documentation skills.
• Record patient progress on weekly notes.
• Collaborate with all disciplines for patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Report to work on time and maintain professionalism.
• Perform duties assigned by Director of Rehabilitation.

Responsibilities

• Carry out treatments under supervision of the OT.
• Provide feedback via documentation or other policy-required methods.
• Collaborate with other disciplines to ensure best patient-centered care.
• Document patient progress in weekly progress notes in a timely fashion.
• Perform all other duties assigned by the Director of Rehabilitation.
