The hospital Quality Data Abstractor and policy coordinator is responsible for collecting and analyzing healthcare data to ensure accurate reporting of hospital performance and patient outcomes. The role involves abstracting clinical information from patient records to meet regulatory requirements and to support quality improvement initiatives. The ideal candidate is a Registered Nurse (RN) with a strong background in clinical practice and experience in healthcare quality management. The abstractor will play a key role in identifying trends, ensuring compliance with national standards and helping to enhance patient care quality.
- Review and abstract clinical data from patient medical records to support hospital quality initiatives.
- Ensure data accuracy and completeness for submission to regulatory agencies such as The Joint Commission, CMS (Centers for Medicare & Medicaid Services), and other accrediting bodies.
- Utilize abstraction tools and software to enter data and generate reports.
- Prepare and submit quality data reports in a timely manner to meet reporting deadlines.
- Collaborates with quality improvement teams to validate and verify data accuracy.
- Assist in the analysis of quality data to identify trends and areas for improvement.
- Ensures compliance with federal, state, and local regulations related to healthcare quality data reporting.
- Stay updated with changes in quality measures, reporting guidelines, and abstraction methodologies.
- Participate in audits and quality reviews to verify the accuracy of reported data.
- Work closely with clinical staff, physicians, and administration to obtain necessary information and clarify documentation.
- Participate in quality improvement meetings and committees to discuss data findings and recommendations.
- Provide education and feedback to clinical teams regarding quality measures and documentation requirements.
- Identify opportunities for process improvement in data collection and reporting.
- Develop and implement best practice for data abstraction and quality reporting.
- Contribute to the development and implementation of quality improvement initiatives and projects.

- Proficient in using electronic health record (EHR) systems and quality reporting software.

EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:
I. Education:
Nursing Diploma or equivalent.
II. Experience:
Minimum of 2-3 years of clinical nursing experience, preferably in a hospital setting. Experience in quality improvement, data abstraction, or healthcare analytics in highly preferred.
III. Software/Hardware:
Microsoft word processing, spreadsheets, graphics, and relations databases required.
IV. Certification/Licensure:
Certification in quality management or related field (e.g: Certified Professional in Healthcare Quality - CPHQ) is a plus.
