Job summary

LakeHouse Senior Living is seeking an LPN to provide nursing care in accordance with each resident’s care plan. You will assess health status, monitor conditions, and coordinate services to promote health and well-being.

The role requires strong communication, organization, and the ability to manage multiple priorities, including nights and weekends. Qualified candidates will have a valid LPN license and at least two years of experience in assisted living or a nursing home setting.

Qualifications

• Two years LPN experience required.
• Two years in assisted living or nursing home preferred.
• Licensed Practical Nurse license in good standing.
• Ability to ensure compliance with federal, state and local laws and regulations.

Responsibilities

• Provide nursing care per the client’s plan of care, including comprehensive health and psychosocial evaluation.
• Monitor client condition and coordinate services as needed.
• Support health promotion and prevention activities for residents.

As an LPN, you will provide nursing care in accordance with the client's plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client’s condition, health promotion and prevention coordination of services.

Qualifications:

• Two years LPN experience required
• Two years’ experience in assisted living or in a nursing home
• Licensed Practical Nurse license in good standing
• Ability to ensure community follows all federal, state and local laws and regulations as pertains to resident services and care.
• Demonstrate good judgment, problem solving and decision-making skills
• Effective organization, time management, and written and verbal communication skills.
• The ability to handle multiple priorities and delegate assignments.
• A dedication to seniors and their well-being is essential.
• The ability to work nights and weekends.
