Job summary

The Metropolitan Jewish Health System is seeking an experienced Registered Nurse for our Home Care team in the New York area. You will deliver advanced clinical home care services, assess patients, manage medications, and provide family education to support safe recovery at home.

Ideal candidates hold an NYS RN license, have at least one year of acute medical-surgical experience, and are bilingual (English/Spanish).

Qualifications

• Current NYS RN license and registration.
• Bachelor’s degree in nursing preferred.
• Minimum of one year acute medical-surgical nursing experience.
• CHHA experience preferred.
• Driver license and car required.
• Bilingual English / Spanish preferred.

Responsibilities

• Map out and oversee the patient’s recovery at home with an interdisciplinary plan.
• Provide advanced clinical home care services: assessment, medication management, and education.
• Coordinate and evaluate the home health plan of care with the physician and care team.
• Review patient insurance information and coordinate care plan activities.
• Demonstrate strong communication, documentation, and time management throughout care.

Responsibilities

MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.

Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient’s recovery from illness, injury or surgery at home.You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.

In this role, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate thehome care needs of your patients. You will also review patient insurance information, coordinate activities involved ineach individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Yourexcellent communication, documentation and time management capabilities will be essential to your success. And allalong the way, you will experience the respect for your opinions and the high level of autonomy you need to do the job

right.

• To create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the home health plan of care in collaboration with the physician
• To effectively case manage the care and services for assigned patients resulting in positive clinical outcomes and minimized risks of rehospitalization
• Supports agency established financial goals
• Demonstrate expert knowledge unique to the practice of community health nursing
