Northwestern Memorial Hospital is seeking a Stroke Program Coordinator to oversee the care of stroke patients. The successful candidate will perform vital duties in compliance with The Joint Commission while ensuring quality improvement and patient satisfaction.

The Stroke Program Coordinator is an experienced clinician who oversees the care of stroke patients. This key team member performs duties in support of and in compliance with The Joint Commission, other licensing/accreditation bodies, and performance improvement plans. Responsibilities include data collection, analysis, continuous quality improvement, patient satisfaction initiatives, and community outreach, as well as patient guidance, education, and follow‑up. The role works collaboratively with staff and physicians to create seamless coordination of care for the stroke patient population.
