Job summary

Centra Healthcare Solutions Inc is seeking an experienced Speech Language Pathologist (SLP) for a contract assignment in a SNF/LTC setting. You will provide skilled speech therapy services in accordance with physician orders while collaborating with an interdisciplinary team to implement the plan of care.

The ideal candidate holds an SLP-CCC, has graduated from an accredited program, and is currently licensed or pursuing licensure in California.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or actively pursuing licensure.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions per physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop or recover communication and swallowing skills.

Responsibilities

• Provide skilled speech therapy services in accordance with physician orders.
• Collaborate with the care team to implement and adjust the plan of care.
• Set measurable goals to improve communication and swallowing abilities.
