Qualifications

• Licensed Physical Therapist in Montana or eligible for licensure.
• Self-directed and able to work with autonomy.
• Willing to commit to a 3–5 year contract with CFVH.
• Experience in rehab or physical therapy settings is preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy interventions and treatments.
• Monitor progress and adjust plans as needed.
• Maintain timely clinical documentation and effective communication with the care team.
• Participate in care coordination and interdisciplinary collaboration.
• Follow CFVH policies and regulatory requirements.
• Maintain licensure and continuing education.
• Contribute to a patient-centered rehabilitation environment.
