Centra Healthcare is seeking a licensed Physical Therapist (PT) for a contract assignment in Cleburne, TX. The PT will provide skilled therapy services, develop and adjust plans of care, and collaborate with the care team to meet patient goals.

Requirements include graduation from an accredited program, current TX PT license or in process, and CPR certification. This is a contract position with a day schedule and weekly pay; Centra covers benefits and reimbursements.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in licensure process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
