Under the direct supervision of the Program Manager, the Consumer Services Coordinator (CSC) is responsible for coordinating the services and supports that are available to persons with developmental disabilities and their families in accordance with the Lanterman Developmental Disabilities Services Act. This position has no supervisory responsibility.

Under the direct supervision of the Program Manager, the Consumer Services Coordinator (CSC) is responsible for coordinating the services and supports that are available to persons with developmental disabilities and their families in accordance with the Lanterman Developmental Disabilities Services Act. This position would provide services to consumers in the Riverside Lower Desert Transition area, which includes driving to Palm Springs, 29 Palms, Thousand Palms, Palm Desert, Rancho Mirage, Cathedral City, Whitewater, Blythe, Morongo Basin, Desert Hot Springs, North Shore, Bermuda Dunes, Thermal, Cabazon, Mecca, Coachella, Landers, Pioneertown, Joshua Tree, Yucca Valley, Indio, La Quinta, Indian Wells. Mileage reimbursement for business travel is paid out on a monthly basis.

Sign-On Bonus
  • $250 sign-on bonus after the completion of 90 days of employment at IRC with work performance in good standing.
  • $500 sign-on bonus after the completion of 6 months of employment at IRC with work performance in good standing.

Essential Duties And Responsibilities
  • Provide case coordination services for consumers as a CSC.
  • Utilize Person-Centered Planning strategies to create and execute the Individual Program Plan (IPP), generate progress reports, and conduct annual reviews.
  • Evaluate consumer progress and plan as identified in the IPP; create and implement IPP Addendums as needed.
  • Provide advocacy services to consumers and families. Attend relevant meetings (IEP, ITP, IHC, etc.) and prepare paperwork as necessary.
  • Utilize generic resources on behalf of consumers and their families.
  • Complete individual or family crisis interventions and document activities.
  • Complete Special Incident Reports promptly and inform appropriate representatives of other agencies and programs.
  • Coordinate consumer assessments (medical, psychological, developmental, educational, vocational, OT/PT, and others).
  • Complete placement and liaison activities as required.
  • Ensure Medicaid Waiver standards are maintained and documentation is timely.
  • Arrange placement of consumers in residential facilities, day programs, and/or other training programs as clinically indicated and approved by the Interdisciplinary Team.
  • Complete facility audits (ICRC 513 form) as required and when facility problems are identified.
  • Identify situations in vendored programs needing attention of Quality Assurance or Resource Development and Transportation programs and make the appropriate referral.
  • Keep the manager informed of caseload status, community activities, and unusual or difficult cases.
  • Participate in case conferences and interagency meetings as needed or assigned.
  • Provide case coordination coverage as directed by the manager when a CSC is absent or there is no assigned CSC.
  • Act as a mentor for new employees as directed by the manager.
  • Complete IRC orientation and new staff training sessions.
  • Visit Level 2, 3, and 4 residential facilities and day programs as assigned.
  • Participate in at least one Quality Assurance evaluation/audit annually.
  • Maintain administrative requirements daily, complete Purchase of Service forms, update consumer files, and document activity in Target Case Management notes (Title 19). Ensure required units and timely completion of paperwork.
  • Facilitate the purchase of services identified in the IPP.
  • Prepare for compliance reviews, eligibility reviews, and other administrative reviews.
  • Maintain and expand knowledge through mandatory and approved trainings to meet CEU requirements.
  • Collaborate as a team, answer questions, share expertise, and contribute to a positive team environment.
  • Manage change effectively and remain adaptable to interruptions and new priorities.
  • Handle a hybrid work environment with office, home, and field work while maintaining productivity.
  • Organize travel to visit consumers, families, vendors, etc. efficiently and effectively.
  • If bilingual, utilize language skills as required.
  • Use office equipment appropriately, keep work area orderly, and comply with safety rules and IRC’s Injury and Illness Prevention Plan.
  • Utilize IRC IT systems and maintain security and compliance with protocols and requirements.
  • Ensure consumers’ rights and dignity are maintained in service provision.
  • Comply with IRC personnel policies and procedures.
  • Perform additional duties as assigned.
Minimum Position Requirements
  • Bachelor’s degree from an accredited college or university and one year of experience (including case management) in intellectual/developmental disabilities, social work, special education, early childhood development, or related field; OR Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or related field.
  • Bilingual preferred but not required.
  • Ability to establish and maintain effective working relationships and to work cooperatively with others.
  • Good attendance and punctuality.
  • Ability to follow oral and written directions; good verbal and written communication skills.
  • Full use of an automobile and a valid California driver’s license with liability insurance; ability to provide independent transportation.
  • Must have and maintain a safe driving record.
